GLP-1 eligibility criteria providers use
- Adults 18 to 74 in one of the 49 states or Washington, D.C. we serve
- BMI of 30 or higher, or BMI of 27 or higher with a weight-related condition such as high blood pressure, type 2 diabetes, or high cholesterol
- Willing to complete a clinical questionnaire and share health history
Who is not eligible
- Under 18 or over 74 years old
- Pregnant, breastfeeding, or planning pregnancy
- Personal or family history of medullary thyroid carcinoma or MEN2
- History of pancreatitis, significant kidney disease, or an active eating disorder
- Louisiana residents — state law bars prescribing compounded medications through this pathway
What happens after you check
